Your 2025 W2 tax form is now available to you on the DecisionHR app. A paper form has been mailed to your home address on record and will arrive before the end of January 2026.
If you haven’t signed up for the DecisionHR Employee App yet, visit https://employee.dhrlive.com/Account/Login using any web browser to register for your account. Once your account is created, you can download the app in your app store:

Your W2 will be located in the “Me” section. Click to the Me section and locate the tab for Tax Forms.
