How do I sign up for DecisionHR's Employee Mobile App?
To use DecisionHR’s Employee Mobile App, you will first need to create an employee account using any web browser and the invite you have received from DecisionHR. If you have not received your invite, please contact using the contact information below.
Once your account is created, you can download the DecisionHR Employee Mobile App, and log in using the same username/password that was used to create your employee account. This convenient app allows you to access your tax forms, pay statements, and other important documents quickly and securely.

Why is DecisionHR asking for personal information?
DecisionHR requires several pieces of information from employees before we can send documents, conduct password resets, or provide any sensitive information. This helps us confirm your identity and acts as a security measure to protect you from fraud and scams. After you have confirmed your identity with our Client Experience Team, none of the information is saved and is disposed of immediately.
1095-C Health Coverage Tax Document Information
The 2024 Form 1095-Cs are prepared and available upon request. Form 1095-Cs provide information about offers of coverage made to full-time employees.
